In case you missed it, the American Society of Transplant Surgeons (ASTS) wrote a Healio op-ed in response to AOPO’s recent piece, taking a deeper look into the complexities surrounding the rise in organ non-utilization. AOPO and ASTS agree that optimizing organ utilization to save more lives necessitates aligning performance metrics among stakeholders within federal policy. ASTS said it well, “OPOs are driven by the number of organs recovered [and transplanted]…and transplant centers are judged by actual patient outcomes,” and “successful minimization of organ nonuse will require a comprehensive approach that incentivizes collaboration, cooperation, trust, research, and mutual learning.” View ASTS’ opinion here. #OrganDonation #OrganTransplant #DonateLife

Ending the week on a high note: in a first-of-its-kind scenario, two Micromates supported different cases at the same time, in two adjacent rooms. While ablations were being performed in one room, lung biopsies were taking place in another - an exceptionally efficient and productive morning for Dr. Marco Van Strijen's team at St. Antonius Hospital Nieuwegein, The Netherlands. Pictured: One of the ablation cases involving the use of three cryoablation needles and an air dissection needle to avoid damaging the intestinal tissues next to the kidney. Prior to the ablation, a kidney biopsy was conducted. New Insights from Our HeartMate 3 LVAD Study at Sheba Medical CenterHey everyone! 👋 I'm excited to share some key findings from our recent research at Sheba Medical Center, focusing on the HeartMate 3 LVAD. A left ventricular assist device (LVAD) is implanted to support the heart's ability to pump blood in patients with severe heart failure.The HeartMate 3 LVAD is not just a piece of technology; it's a lifeline for those with advanced heart conditions, offering a bridge to heart transplantation or serving as long-term support. Our study, from 2016 to 2022, included 131 patients and examined whether undergoing additional cardiac surgeries during LVAD implantation affected survival rates. Our findings showed that survival rates were similar across both groups. This indicated that performing additional procedures during LVAD implantation was as safe as the LVAD implantation alone.A huge shoutout to Jonathan Eisenberger, whose leadership, dedication, and understanding were invaluable in conducting this research.

He graciously shared with me the incredible story of his pioneering work in liver transplantation, a tale that not only highlights the pivotal role perseverance plays in a surgeon's life but also takes us on a nostalgic journey through the chapters of medical history.Back in 1963, Prof. Starzl faced daunting challenges and initial failures in his quest to perfect liver transplants. For almost 100 years, it has been known that arginine is a conditional amino acid (that is, requiring replacement under certain states). What is new however, is that arginine deficiency, caused by its pathologic destruction by myeloid derived suppressor cells (MDSC) can significantly worsen the prognosis of different illnesses including patients with certain tumors. Patients with arginine deficiency undergoing surgery for cancer are particularly vulnerable to the development of postoperative infections, wound breakdown, and other complications. Arginine Replacement restores cellular and organ physiology resulting in improved clinical outcomes. According to reviews and guidelines sponsored by different professional societies, perioperative arginine replacement should be considered in patients undergoing gastrointestinal surgery for cancer. Evidence of the benefits of arginine replacement is also growing in patients undergoing surgery and adjuvant therapy for head and neck cancer and in patients with hemolytic diseases. Side effects and contraindications to arginine replacement do exist and, like any other medical intervention, its use should follow the scientific evidence of benefit and a careful analysis of risks.

Pressurized IntraPeritoneal Aerosol Chemotherapy (PIPAC) is a great adjunct in the oncologic surgeon armamentarium in cases with peritoneal carcinomatosis. Worth exploring other possible applications such as locally advanced abdominal cancers without peritoneal carcinomatosis after resection. PIPAC is routinely offered by the Digestive and Emergency Department of the S.Maria Hospital, Terni, along with PITAC (Pressurized IntraThoracic Aerosol Chemotherapy) and HIPEC (Hypertermic IntraPeritoneal Chemotherapy) with or without peritonectomy, to patients who fulfil the criteria for those treatments and who can benefit from them.#chirurgiadigestivaterni #pipac #pitac #hipec #peritonealcarcinomatosis

C-Path's #TransplantTherapeuticsConsortium is pleased to announce that “Applying propensity methods to the United States transplant registry for external real-world evidence control arms for 5-year survival in the BENEFIT study” has been published in American Society of Transplant Surgeons and American Society of Transplantation's American Journal of Transplantation. This study was a collaboration between the TTC and United Network for Organ Sharing (UNOS). This study expands the concept of applying registry data as external controls to supplement internal controls in regulatory decision-making under the FDA accelerated approval program, which would require long-term confirmatory survival follow-up for a new therapy approved using a RLSE.You can read the manuscript in its entirety here.
